The Family Educational Rights and Privacy Act is a Federal law that protects the privacy of student education records. The law applies to all schools that receive funds under an applicable program of the U.S. Department of Education.

FERPA gives parents certain rights with respect to their children’s education records. These rights transfer to the student when he or she reaches the age of 18 or attends a school beyond the high school level. Students to whom the rights have transferred are “eligible students.”

By signing this form I understand that I am authorizing Denmark Technical College to release the indicated information to the party specified above. Once released this information may no longer be protected by FERPA. This agreement will remain in effect for until I graduate, am no longer a student at DTC or until the canceled it in writing at the Office of the Registrar. Student must submit ID at time of request.

I will not be eligible to receive other Title IV aid, which includes the Pell Grant, Federal Supplemental Educational
Opportunity Grant, Perkins, Loan, Stafford Loans, federal work study and the SC Need-based Grant.
Also, I will not be able to participate in other loan programs offered or other state assistance programs that require the submission
of the FAFSA.

Any false information provided by me or any attempt to receive scholarship funds for unlawful purposes and/or for any purpose other than for payment of my incurred cost of attendance at Denmark Technical College will be cause for immediate cancellation. Any student who willfully obtains a scholarship through means of a false statement or failure to reveal any material fact, condition, or circumstances that could affect eligibility will be subject to applicable civil and/or criminal penalties, including retroactive loss of the scholarship or grant.

As a SC State Grant or Scholarship recipient, I certify that:

· I am a legal resident of South Carolina and a US citizen or legal permanent resident.

· Graduated [or will graduate*] from a high school in SC or successfully [or will successfully*] completed a homeschool program as prescribed by law or graduated [will graduate*] from a preparatory high school outside SC, while being the dependent of a parent or legal guardian who is a legal resident of SC and has custody of me.

· I have not been adjudicated delinquent, convicted, or pled guilty or nolo contendere to any felonies or any second or subsequent alcohol or drug related offenses under the laws of this, or any state, of the US within the past academic year.

· I have not defaulted, do not owe a refund, nor am I in repayment on any federal or state financial aid.

Any LIFE Scholarship recipient student with disabilities may be approved to take a reduced course load to meet the full-time enrollment requirement and annual credit hour requirement. The approval must come from DTC’s Career and Student Success Department prior to the last day of the Add/Drop period. Please contact the Career and Counseling Department at (803) 793-5287.

As a part of being enrolled in dual credit courses through Denmark Technical College (DTC), I agree to the following:

1. I have been through the D2L orientation and know how to navigate my courses and access my grades.

2. I understand the impact my grades in the dual-enrollment classes have on both my high school and college GPA.

3. I understand that my grades in the dual credit courses will be calculated into my college GPA at the end of my freshmen year in college. If these grades cause my GPA to drop below a 3.0, then I could lose scholarship money.

4. If I am having any issues with my dual credit courses, I will notify my Professional School Counselor, immediately. I will also communicate with my DTC instructor and the Dual Enrollment Coordinator, if I am having challenges.

5. I understand the importance of logging into my courses daily AND submitting assignments on time.

6. I understand that DTC tracks students logging into D2L accounts and keeps account of whether a student is completing assignments.

7. I understand that I must be enrolled in a minimum of six (6) credit hours to be eligible for Lottery Tuition Assistance (LTA).

8. I understand that if I do not login to and complete an assignment in my course within the first five days of the course, I will be dropped from the course and the course will not appear on my transcript.

9. Withdrawal Requests:

1) If I request to be withdrawn from a course(s) before the last day of the midterm week, a grade of “W” will be assigned and will appear on my transcript. This grade will not be calculated in my grade point average.

2) If I request to be withdrawn from a course(s) on or after the last day of the midterm week , I will be withdrawn and a grade of “WF” Withdrawn Failing or “WP” Withdrawn Passing will be assigned at the discretion of the instructor and will appear on my transcript. If I earn a grade of “WF”, it will be calculated in my grade point average as zero grade points.

10. I understand that if I am dropped from a course for not attending or request to be dropped from a course that places me below six (6) credit hours, I will NO LONGER be eligible for

LTA and will owe a balance to the College of $188.25 per credit hour that will be due immediately.

11. I understand that by becoming a Dual Enrolled student I must abide by the high school’s and the college’s rules and regulations. As a dependent of my parent, I give DTC permission to release any information regarding my grades, attendance, academic standing, disciplinary action, financial obligations, or any other aspect of my student life to my parents or legal guardians. I also understand that grades, attendance, and any other academic information will be provided to the high school as part of this agreement.

The last date to drop/add a course is August 18, 2023, at 5:00 pm for the 2023 Fall Semester.
